OHIP doesn’t pay for all of the services your doctor provides to you. Our physicians can request payment from patients for the services that are not covered by OHIP.

Uninsured Medical Services:

​

$60        Private Visit (General Consult without OHIP card)

$120      Physical Check-Up (without OHIP Card)

$40        TB Skin Test (Employment Purpose)

$10        Call-in Prescription renewal orders to the pharmacist.

Non-Medical Services

​

$20         Sick Note (Back to work note / Daycare Note)

$25         Completion of Physical Forms for pre-employment certificate /placement forms

$35          TTC Sick Benefits Form

$35         Application for Accessible Parking Permit

$50          Missed Appointments (specialists & in-office Procedures)

$75          Revenue Canada, Federal Disability Tax Credit

$85          Transfer of Medical Records

$85          CPP disability Medical Report Form

$100        MOT Driver’s Medical Exam

$125        Completion of Disability Form (OCF-3)

$150        Citizen and Immigration Canada Medical Report for Immigration

$250        Insurance Medical Examination (including assessment and report)

$35 - $150   other forms not covered by OHIP; Fee is determined by Physicians.
